“On January 1st, 2021, I had a Major Ischemic Stroke leaving me with limited to no movement (paralysis) on my entire left side. Due to the shutdowns with COVID, my age and the severity of my condition it was “highly suggested” that I be released to a LTC facility. Fortunately, my family knew that mentally and emotionally I was not ready for that move. Since I had been highly functionally and independent 82-year-old woman the day before my stroke. Although I have several family members with healthcare backgrounds; Celeste’s compassionate guidance and problem-solving skills, contributed to me living back at my home with a caregiver, a year and a half later. She encouraged my family, to apply for the Family Managed Home Care program with the HCCSS. Once accepted, this program allows for me to direct my care (how and when I need it). She also suggested providers that could meet my care needs under this program’s restrictions (providing consistency). It was with her insights on managing in-home care, that positively changed the where and how for me. Enabling my family and I to manage outside of LTC.
